Tell me about a mistake you made that someone else needed to hear about. Who told them — you or somebody else — and when?
Why this works
Everyone makes mistakes worth reporting; the score is in the gap between knowing and telling, and in who did the telling. A rehearsed answer can polish the mistake, but not the timeline — the probes walk it hour by hour.
Follow-up probes
- What did you know at the moment you decided to speak — or wait?
- What did they hear first — the problem, or your plan for it?
- What did the fix cost, and who carried that cost?
- What makes the same mistake harder to make today?
What good looks like
They disclosed it themselves within hours or days of knowing, arrived with a repair plan rather than an apology alone, sized the damage honestly without minimizing, and changed something concrete so recurrence got harder.
Red flags
The mistake surfaced through someone else's discovery, the example is trivially safe, responsibility dissolves into "we" the moment damage comes up, or the fix was silence plus hope.
